Primary Health service is committed to supporting clients to access the health care services that they need, ensuring that clients receive appropriate care and support to maintain their independence, health and well-being to remain living at home.My Aged Care is the Australian Government’s starting point for clients to access government-funded services, including community based district nursing, allied health, social support groups and residential aged care.
Our Primary Health department have coordinated a series of education sessions to support staff in their understanding of My Aged Care and how to support clients in the navigation of this on-line system. Staff gained knowledge of the services available, the referral pathways and how to support clients to access assessment and services. Through our Intake service we have supported many community members to register with My Aged Care and commence their journey to access these government funded services.
