What is A Diabetes Educator?
A Diabetes Educator works with people who have or are at risk of developing diabetes to improve understanding and better manage diabetes and their general health and wellbeing. We assist you to focus on your individual needs, providing knowledge, motivation and support to assist in preventing diabetes related health complications.
When can I contact the Diabetes Educator?
Mondays, alternate Wednesdays, or Thursdays (except public holidays)
8:30 am - 5:00 pm
How can I arrange to see the Diabetes Educator?
Most people get a referral from their doctor (GP) but you are welcome to speak with our Intake Coordinator who will help arrange an appointment to see the Diabetes Educator
How much does it cost?
Health Care Card/Pension Concession Holders - $10.10 per visit
Medium-High Income - $15.50 per visit
Other payment options are available.
Make a booking
When we receive your referral our Intake Coordinator will contact you about booking an appointment. Alternatively you can contact the Primary Health department on (03) 5862 0560 or in person at the Primary Health Reception, 2 Katamatite Road, Numurkah. Our Intake Coordinator will then talk to you about your request.
The visitor car park is located on Katamatite Road.
