What is a Chronic Disease Worker?
Our Chronic Disease Worker supports people with chronic illness such as arthritis, heart disease, lung problems like asthma, and diabetes to better look after their health. They assist their clients to set their own goals and work towards achieving them to improve their health and wellbeing.

Tuesdays and every second Monday (except public holidays)
8:30 am - 5:00 pm
How can i arrange to see the Chronic Disease Worker?
Most people get a referral from their doctor (GP) but you are welcome to speak with our Intake Coordinator who will help arrange an appointment to see the Chronic Disease Worker.
How much does it cost?
Health Care Card/Pension Concession Holders - $10.10 per visit
Medium-High Income - $15.50 per visit
Other payment options are available.

Make a booking
When we receive your referral our Intake Coordinator will contact you about booking an appointment. Alternatively you can contact the Primary Health department on (03) 5862 0560 or in person at the Primary Health Reception, 2 Katamatite Road, Numurkah. Our Intake Coordinator will then talk to you about your request.
The visitor car park is located on Katamatite Road.
