Chronic Disease Workers are Allied Health professionals that help people with chronic disease look after their health. They assist their clients to set their own goals and work towards achieving them.

Karen Edwards is a trained Quit Educator, Anaphylaxis Trainer, Asthma Educator and Health Coach.
When can you see the Chronic Disease Worker?
Tuesdays and every second Monday (except public holidays)
Who can see the Chronic Disease Worker?
Most bookings (referrals) come from the doctor (GP) but you are welcome to speak with our Intake Team who will help direct you to the service that best suits your needs.
How much does it cost?
Health Care Card Holders - $9.60 per visit
