What is the Program?
The Early Intervention in Chronic Disease (EIiCD) program adds to the work we already do to help people in the community living with chronic disease look after themselves better. It aims to get in early to support people before their health problems become more complex.
The program is funded by the Victorian Department of Health and Human Services. Primary Health services use this extra funding to improve the way they do day-to-day work. They also find ways to work better together with other services to make sure that your journey through the health care system is as smooth as possible.

Our local approach in the Moira Shire
In the Moira Shire, this program is managed by our Chronic Disease Community Worker, Karen Edwards at Numurkah District Health Service. Karen works with the other primary health services across the Shire to make sure we are all working together as best as we can to support people with chronic disease.
